Letter of Authorization

Local Number Portability (LNP) allows you to move your current telephone number from your current telephone service provider to OurOldGroup, LLC dba OurOldNumber.com (OOG). Your current provider requires this Letter of Authorization (LOA) as proof that you have explicitly requested and authorized OOG to transfer the telephone number(s) you’ve indicated below. By submitting this form, you authorize us to initiate the process of transferring these telephone number(s) to OOG. After the transfer is completed, your transferred telephone number(s) will be part of your OOG account. Once this form has been returned to OOG, the number porting process cannot be stopped. However, you retain ownership of this number and you can port it back or to another carrier at any time.

The process of transferring your telephone number(s) will typically take a minimum of seven (7) business days to complete. During this time, your current telephone service will not be interrupted. Once the change has taken place, calls to your current telephone number(s) will ring to your OOG service. Please DO NOT take any steps to cancel your existing landline service with your existing provider until you have been notified that your new service with OOG is complete. Canceling your existing service after submitting this form and before the number port is completed could result in losing your telephone number(s).

Again, please note that the information you entered MUST match the information that your current provider has on file. This information will appear on the front page of your current provider’s invoice.
 
If the number you are transferring is a cellular or hybrid telephone number, please provide both the account number and pin code associated with the account.
I designate OurOldGroup, LLC dba OurOldNumber.com (OOG) or it’s designated agent to transfer my telephone numbers(s) from my current provider to OOG and authorize OOG to obtain billing information, customer service records or any other information from my current provider that will aid in transferring my telephone number(s) to OOG.
